Canadian Center for Vaccinology Pharmacists as Immunizers Survey

Who is doing the study?
This study is being conducted by the Canadian Immunization Research Network by researchers at the Canadian Center for Vaccinology at Dalhousie University in Halifax. The lead researchers are Dr. Jennifer Isenor, Dr. Scott Halperin, Dr. Donna MacDougall, and Dr. Karina Top along with student investigator Antonia Di Castri.

What is the purpose of this research study?
The purpose of the study is to determine the knowledge, attitudes, beliefs and behaviors of the healthcare providers regarding the delivery of vaccines for adults by pharmacists. The goal of the survey is to determine knowledge about specific vaccines, current and changing vaccine practices, and logistics of vaccination delivery.
What will be done?
You will complete this online survey. It will take less than 10 minutes to complete. The survey asks questions about you, the pertussis (whooping cough, Tdap) vaccine, the meningococcal B and ACWY vaccines, the high-dose influenza vaccine and your opinions about current vaccination programs. Your response is important.  
Potential harms and burdens:
There are no expected harms although it is possible some of the questions may cause some distress. Taking part will be of no benefit to you personally. It is hoped that what is learned will be of future benefit to others.  
Costs and reimbursements:
There will be no costs for time spent completing the survey. As a token of our appreciation for completing this very important survey, you will be invited to enter your email address for a chance to win a $100 gift card to a restaurant of your choice. Please note this prize draw is completely optional. If you choose to participate your email address will be stored in a separate dataset, which will not be linked to your survey responses.
How will my privacy be protected?
Your responses will be kept completely confidential. Your responses will not be seen by anyone other than the research staff. The research staff will keep all information that is learned about you confidential. To do this, a code number will be assigned to your study information.  
Decision to withdraw at any time:
Taking part in this research study is up to you. If you do not want to continue, you can leave this website at any time. You may not withdraw from the study after submission of the survey.

What are my Research Rights?  
Completion of the survey indicates that you have agreed to take part in this research and for your responses to be used.  In no way does this waive your legal rights nor release the investigator(s) or involved institution(s) from their legal and professional responsibilities. How the findings will be used?
You will not be identified by name in any reports or publications. The results of the study may be used for public health program planning and decision making. At the end of the study a summary of the results will be presented at professional conferences and will be published in professional journals.
